When providing services to the poor of our community with limited resources, value-based partnerships are crucial. Value-based partnering recognizes the interdependencies among stakeholders in the health care system and creates a strategic reason for them to exchange information and create strategic alliances. The Rio Grande State Center Outpatient Clinic works strategically with its partners to expand services and improve access to care for those who would otherwise go unserved. The partnership with Methodist Healthcare Ministries' community-based counselors has transformed the delivery of primary care, promoting and supporting behavioral health as a vital component of the physician-patient encounter.

The session will review the three-year evolution of the interprofessional collaborative care unit (ICCU), which was developed to provide care to adolescents ages 12 to 18 triply diagnosed with a mental health, medical health, and appetitive drive disorders. The evolution begins with a simulation, development of interprofessional care, and the beginning of integrated care.
